Broxtowe Health Partnership Lifestyle Fund Autumn 2015

Through the Broxtowe Health Partnership we are once again inviting local groups and organisations to put forward proposals for projects that will take creative approaches to improving health locally. A maximum of £2,500 can be applied for per project (applications are limited to one project per organisation). Successful projects will promote health and well-being by identifying and addressing inequalities in health or the factors that cause ill-health.  Projects can cover a number of themes and deliver in one or more geographical areas within the Borough of Broxtowe. We are particularly keen to support projects which target the areas of highest need..

 

What types of projects are likely to be successful?

In previous years, a wide range of projects have been supported, including Walk & Talk groups, Sing & Smile classes, Healthy Lifestyle Outreach Events, Bring Baby Gym, A Friendly Place To Go friendship group, Support Our Forces activities, cycling for all, domestic violence drop-in support groups, and many others.

 

Please outline your proposal using the application form attached (preferably electronically) and return it by 5pm on Thursday 15th October 2015 to

nottingham.west@nottinghamwestccg.nhs.uk  An acknowledgment of receipt of your application will be sent to you.

The Lifestyle Fund, which is in its third year, has been provided by NHS Nottingham West Clinical Commissioning Group to support local initiatives that aim to improve health and reduce health inequalities for the residents of Broxtowe.
